An annual physical exam and acknowledgment forms are required in order for your child to participate in Waller ISD (WISD) athletics (7th-12th), band (9th-12), dance (9th-12th), and/or cheer (7th-12th) during the 2023-2024 school year.

The following required forms are available online at https://wisd.fyi/athleticforms (Rank One):

• Waller ISD Athletics Pre-Participation Forms
• Emergency Medical Information & Consent Form
• Special Medical Information Form
• Parent/Guardian Consent Form
• Pre-Participation Medical History - This is the ONLY form that MUST be a paper copy. NO OTHER physical examination form will be accepted as per UIL rules.
